- As a Physician, you will be responsible for coordination of proactive care for patients with chronic conditions as well as identifying wellness and/or preventative service for clinical patients
- Completed Residency Program
- Board Certification in Family Medicine
- Indiana State Medical License, DEA, and CPR certifications
- Ongoing Continuing Medical Education (CME)
- 1-3 years of experience as a physician preferred (but not required)
