Qualifications
- Open to candidates completing a fellowship or a practicing Hematology Oncology physician
- Board certification or board eligibility in Hematology and Oncology
- Current (or eligible), unrestricted Florida medical license (M.D. or D.O.)
- Active state and federal controlled substance registrations
