Qualifications 

• 5+ years of clinical experience
• Board certified or board eligible in Pain Management
• Interventional Pain Physician, Anesthesiologist, Anesthesia-Pain, PM&R - Physical Medicine and Rehabilitation (PM&R) or Neurologist
• MD or DO with ability to acquire Illinois licensure
