Doctor of Medicine (MD) or Doctor of Osteopathic Medicine (DO) from an accredited medical school.
Board Certified or Board Eligible in Dermatology.
Completion of a dermatology residency program.
Active, non-probationary medical license to practice in Arizona.
Valid prescriptive authority.
